We Come to You
Mobile Primary Care, LLC provides quality primary healthcare to the home-bound and home-limited and offers convenient, competent, and compassionate medical care to
you or your loved one. We see patients with simple or complex health issues in private homes, assisted living facilities, and group homes.
We work with patients and their families to provide quality primary care, prescription management and refills, referrals to home health services, and hospice care if needed. We collaborate with your existing healthcare teams and offer blood draws, urinalysis, and referrals for X-rays and EKGs done in the home.
Our goal is to prevent frequent emergency room visits through regular home visits and provide a proactive approach to care coordination.
No more difficult and challenging trips to the doctor--we can take care of all your medical needs from the comfort of your own home!
